FormBuilder Rx for Visit Registration
Attention:
For the new "FormBuilder Rx for Visit Registration" feature to work,
you will need the 2022 Release 1 version of FormBuilder Rx (6.13.2 or higher) and Events
Rx (6.6 or higher). FormBuilder should be installed or upgraded first.

Creating a Visit Registration Page in Page Builder
- Navigate to the FormBuilder Rx tab in FormBuilder Rx (the Environments List
page)
- Click on the Page Builder tab below the Environments
List
- Click on the Create New Page button
- In the Create New Page dialog, select the Page Type as
"Visit Registration Page" from the drop-down list and fill out the other
details
- Click Save
- A new page will be created and appear in the list of pages (in this example
we're using "Demo Visit Page" as the page name)
- Next, click the Configure button to open the settings for
your new Visit Registration Page
- Double click on Activities
- This will open the Activities Configuration dialog
- Select a Default Sorting Order. The two options are: By Timeline and By Activity
- Click Save
- Open the Primary_ContactDS and Guest_ContactDS and assign a Duplicate Matching
Rule:
- To auto-populate the lead source on a new Contact record when using FormBuilder Rx
for visit registration:
- Add the LeadSource field to the Primary Contact and Guest Contact data sets
- Add each field to the appropriate section of the registration page (primary or guest information sections)
- In field configuration, set each to Rendered=FALSE
- Auto populate the Primary contact LeadSource field with the Visit Series
New Contact Lead Source field:
- Auto populate the Guest contact LeadSource field with the Visit Series

Note:If you have FormBuilder Rx, visit the Page Builder section in the FormBuilder Rx product guide for additional information on adding fields to your form. - There is an additional step for existing Visit Registration Pages with the
FormBuilder maximum registrations = 0 feature. You need to manually add a condition for
the "Available Places" paragraph as shown in the following images:
Attention:If Maximum Registrations is set to 0, Maximum Wait-List Registrations must also be set to 0.
Configure the New Visit Series
- Navigate to the Events Rx Admin Panel and click on the Events Calendar icon
- On the Events Calendar page, click the New Visit Series button
- In the new Visit Series, you should now see a dropdown list for FormBuilder
Pages
- Select a page from this field
- Click Save
End User View
Follow these steps to preview how this feature appears to your end users.
- Navigate to Setup and search for "Sites" in the Quick Find box
- Click on the Site URL
- Click on Visit Series
Considerations and Limitations
- The checkbox must be checked for ERxFB for Visit Series Registration in Custom Settings so that the picklist field named "FormBuilder Pages" can appear in Events Rx new Visit Series configuration
- Aggregated fields like Mailing Address and Shipping Address cannot be used in any Data Set of FormBuilder Pages
