Build Your Own Template

Steps to Create a Custom Template

1. On the FormBuilder Rx tab, select the Environment you want to work on (the current Environment's name will appear at the top of the page).
2. Click the Template Builder subtab, then click the New Template button. This will launch a popup.
   
   

   User interface is related to 2025 Release 1.

3. In the "New Template" popup, select "Build Your Own" and click the Proceed button.
   
   A dialog window with options will appear.
   
   
4. Enter a name for your template and optional description, along with HTML for your header and footer.
5. Click Save to close the dialog.
   Tip:

   You can also "Quick Save" in this dialog as you go along.

From here you can add static resources, set display criteria, work with template components and CSS, just as you would with one of our built-in templates.
